The head of the department hematologist, Candidate of medical Sciences, Yana K. Mangasarova.
The Department of Chemotherapy of Lymphatic Tumors with Bone Marrow and Hematopoietic Stem Cell Transplantation Unit with Day Hospital functions as part of the Division of Lymphoproliferative Diseases.
The Department of Hematology and Intensive Care was established on the initiative of Academician of the Russian Academy of Sciences, Professor, Doctor of Medical Sciences Andrey I.Vorobyev. The director of the department was Doctor of Medical Sciences, A.V. Pivnik (since 1990), and the heads were A.M. Kremenetskaya (since 1990) and Candidate of Medical Sciences, S.K Kravchenko (since 2001). In April 2022 Department of Chemotherapy of Lymphatic Tumors with Bone Marrow and Hematopoietic Stem Cell Transplantation Unit with Day Hospital opened on the foundations of the Department of Hematology and Intensive Care. During its existence, under the leadership of A.I. Vorobyov. the foundations were laid for the diagnosis and treatment of lymphoproliferative diseases of T and B-cell origin, pathology of red blood, thrombotic complications, lymphomas in pregnant women, porphyrin metabolism disorders, hairy cell leukemia, and skin lymphomas.
The department has 19 specialized hematology beds, including 1-bed and 2-bed wards.
List of diseases and syndromes with which patients can be referred for examination and treatment:
– diffuse large B-cell cell lymphoma
– double/triple hit-lymphoma
– double/triple expressor-lymphoma
– primary mediastinal large B-cell cell lymphoma
– Unclassifiable B-cell lymphoma occupying an intermediate position between DLBCL and classical Hodgkin's lymphoma (mediastinal gray zone lymphoma)
– anaplastic large cell ALA-positive/negative lymphoma
– peripheral T-cell lymphoma
– angioimmunoblastic T-cell lymphoma
– extranodal NK/T-cell lymphoma
– fungal mycosis and Sezary syndrome
– primary cutaneous CD30-positive lymphoproliferative diseases
– B/T-cell lymphomas of the skin
– follicular lymphoma
– Burkitt's lymphoma
– lymphoma from marginal zone cells
– classic Hodgkin's lymphoma
– other rare forms of lymphoproliferative diseases
The main objective of the clinical and research activities of the department is to study the biological phenomena underlying lymphoproliferative diseases and, based on this, to develop a scientifically based approach to treatment, create new treatment programs and introduce them into broader hematological practice. The scientific work carried out in cooperation with leading laboratories allows the introduction of innovative drugs into therapy protocols, which is supported by grants and is the grounds for many Candidate and Doctoral dissertations.
The department is the only specialized hospital in Russia in the "hematology" profile, designed for chemotherapy in pregnant women with lymphomas. The basic concept of "saving two lives" was laid down by Academician of the Russian Academy of Sciences, Professor, Doctor of Medical Sciences, V.G. Savchenko. The staff of the department have unique experience in managing such complex cases. The work is carried out jointly with leading obstetricians-gynecologists and neonatologists of Moscow.
For the first time in Russia, a group of multicenter cooperative studies on the treatment of diffuse large B-cell lymphoma and primary mediastinal large B-cell lymphoma with the participation of hematology departments of various subjects of the Russian Federation has been initiated. The introduction of an innovative and personalized approach to the treatment of patients with aggressive lymphomas makes it possible to improve survival and neutralize an unfavorable prognosis.
One of the scientific and therapeutic tasks of the department is a wide range of clinical studies on the therapy of nodal T-cell lymphomas, namely, a randomized prospective study in the therapy of anaplastic large-cell ALK-positive lymphomas, a prospective study to evaluate the effectiveness of high-dose therapy programs for ALCL ALK-negative lymphoma, and alternative treatment regimens for peripheral T-cell lymphoma (including the use of autologous hematopoietic stem cell transplantation), angioimmunoblastic T-cell lymphoma, extranodal NK/T-cell lymphoma.
The department conducts research in the field of modern diagnostics of cutaneous lymphomas, the development of the latest, relevant and effective algorithms for the treatment of late stages of fungal mycosis, primary cutaneous CD30-positive lymphomas, and B-cell lymphomas of the skin.
The staff of the department use a completely new stratification method for follicular lymphoma by categorizing the patients into risk groups, which allows identifying an unfavorable group, and taking this into account, conducting patient-specific therapy.
The department has tested and introduced into clinical practice a highly effective protocol for the treatment of patients with refractory and recurrent Hodgkin's lymphoma. The therapy is based on a combination of immune checkpoint inhibitors with chemotherapy and autologous hematopoietic stem cell transplantation.
Programs of rational precision and empirical antimicrobial therapy of infections in neutropenia have been developed and are being improved upon based on the study of the spectrum and antibiotic sensitivity of pathogens isolated from patients with lymphoproliferative diseases during high-dose chemotherapy and transplantation of autologous hematopoietic blood stem cells.
Every year, the department performs about 1,000 courses of chemotherapy and 40 autologous stem cell transplants as part of the provision of high-tech medical care for citizens of the Russian Federation, as well as citizens of foreign countries on a contractual basis.
Pedagogical work is continuously carried out in the department. During the year, about 20-30 printed works are published on the topics of scientific and clinical works.
The department's staff annually conducts more than 300 consultations within the framework of providing medical care using telemedicine technologies, participates in the development of Russian clinical guidelines for the diagnosis and treatment of lymphoproliferative diseases, constantly presents unique proprietary data at domestic and foreign conferences, participates in research protocols and early access programs. The employees are experts of the Ministry of Health of Russia, the Russian Academy of Sciences and members of international and domestic medical scientific societies in the field of hematology and related disciplines, and hold patents for inventions.
The staff of the department is ever-expanding with active, purposeful and talented specialists who successfully combine scientific work and clinical practice. The realization of all complex routes is possible only with teamwork, the purpose of which is to cure the patient.
