Indications and contraindications for echocardiography (ECHO-KG)
Indications for transthoracic ECHO-KG:
- congenital heart defects;
- acquired heart defects;
- infective endocarditis;
- acute coronary syndrome;
- coronary heart disease;
- heart failure;
- pulmonary embolism;
- pulmonary hypertension of unspecified etiology;
- cardiomyopathy;
- myocarditis;
- pericarditis;
- syncope conditions of unknown etiology;
- suspected heart tumor;
- screening of close relatives of persons who died suddenly or at a young age;
- signs of heart failure;
- screening examination of athletes participating in competitions;
- planned cardiac surgery;
- after invasive procedures (coronary angiography, balloon angioplasty of the coronary arteries).
Indications for dynamic ECHO-KG:
- evaluation of the effectiveness of drug therapy;
- evaluation of the effectiveness of cardiac surgery;
- a sharp change, deterioration of the clinical course of the disease.
Need for monitoring:
- the size and function of the left ventricle (with heart failure, diffuse myocarditis);
- the severity of valvular stenosis;
- systolic pressure gradient (with hypertrophic cardiopathy);
- pulmonary hypertension (for example, with a hemodynamically significant defect of the interventricular, atrial septum).
Indications for transesophageal ECHO-KG:
- identification of sources of systemic embolism;
- evaluation of the structure, size, degree of mobility, place of attachment of neoplasms of the heart;
- diagnosis of bacterial endocarditis;
- diagnosis of diseases of the thoracic aorta (dissecting aortic aneurysm, atherosclerotic lesions of the aorta);
- diagnosis of congenital heart defects;
- evaluation of the condition of the valvular apparatus of the heart (leaflets, subvalvular structures, careful assessment of the degree of regurgitation) with acquired heart defects, including before the forthcoming operation;
- assessment of the dysfunction of a prosthetic valves.
Contraindications for transesophageal ECHO-KG:
- pathology of the esophagus: tumors, diverticula, fistulas, strictures, esophagitis;
- bleeding from the upper gastrointestinal tract.
