Developed in full accordance with the Order of the Ministry of Health of the Russian Federation dated 31.07.2020 No. 789н "On approval of the procedure and terms for the provision of medical documents (their copies) and extracts from them."
Dear patient (patient’s official representative)!
If you need to obtain medical documents (or copies thereof) and extracts from them, please read this memo carefully.1. Medical documentation is issued upon written application from the patient or his representative in accordance with Attachment No.1 (download pdf)..
The application can be submitted to the office in writing and provided:- directly (in person) by the applicant or his/her legal representative;
- by post (for example, Russian Post or other services);
2. Upon receipt of medical documents, the patient must present a passport. The patient's legal representative is obliged to provide a passport and a document confirming his/her status.
3. The applicant can be provided with originals, copies of medical documents, extracts thereof. The originals of the following medical documents are not provided:
- outpatient medical chart (form No. 025 / y);
- inpatient medical chart (form No. 003 / y);
- medical chart of a dental patient (form No. 043 / y);
- autopsy report (form No. 013 / y).
4. Copies of medical documents and extracts are provided to the applicant in the amount of 1 (one) copy.
5. Medical documents (copies, extracts from them) destroyed due to their expired storage period are not provided.
